บอจ.5 is the shareholder list: every shareholder with their shares, filed at incorporation and again after every change, and refiled with the DBD each year after the annual general meeting. It is the document that proves who owns a Thai company.
When a bank, embassy, lawyer or buyer asks who owns the company, the current บอจ.5 from the DBD is the answer they will accept. It is also where the Thai and foreign shareholding split is visible, which is why it matters for the Foreign Business Act and for work permit files.

What the form itself says
- I certify that these entries are correct and match the register of shareholders.
- Note: in the column "Amount paid (1)", state the amount paid up on each share, only for shares that must be paid for in money.
- In the column "Deemed paid (2)", state the amount deemed paid up on each share, only for shares that must be paid for with property or labour.
- In the column "Nationality (3)", if the shareholder is a juristic person, state the country in which it was registered.
